Dedicated expertise: Cognitive Outcomes Leadership
A distinctive part of Fortava’s approach is the implementation of a Cognitive Outcomes Lead, here to ensure that every resident’s care is guided by evidence based practice, structured outcome measures, and measurable improvements in wellbeing.
As part of this commitment, we work in partnership with Brain+ and utilise their Ayla cognitive assessment and therapy system, enabling us to deliver consistent, high-quality Cognitive Stimulation Therapy (CST) and track its impact with exceptional precision.
Our Cognitive Outcomes Lead…
- Provides specialist oversight of assessment and care planning.
- Tracks and evaluates residents’ progress using structured outcomes and Ayla-generated data.
- Mentors staff to deliver CST and other interventions that make a tangible difference.
Having this role—and the advanced digital tools that support it—ensures the care we provide is not only compassionate, but purposeful, data-led, and able to demonstrate clear, positive outcomes for every resident.
Referrals
We aim to make our referral process as straightforward as possible for families, commissioners, and professionals.
We regularly receive referrals from local authorities, NHS partners, and a multitude of other professionals seeking specialist care options – but regardless of where it starts, our team works hand-in-hand with every referrer to ensure continuity, transparency, and the best possible outcome for the individual at the end of the request.

First conversation
Our dedicated referrals team will gather background details about the person needing support.
Step 2
Assessment
A member of our clinical team will carry out an assessment – usually within a few days – to better understand the person’s specific needs and preferences.
Step 3
Next Steps
Working closely with funding partners, we will provide clear recommendations and agree on the most appropriate plan for the prospective resident.
Step 4
Admission
When everyone feels prepared, we create a personalised admission care plan, support a smooth transition, and warmly welcome the individual into our community.
